What information is collected for termination verifications?

Whenever a termination needs to be verified with the employer, they will receive an email containing a summary of the information provided by the carrier and what needs to be confirmed, including:

• Employee name
• Coverage termination reason
• Date coverage terminates
• Address where continuation of coverage forms should be sent, if applicable
